You can enter an existing Medical Center Application Id in the Application ID Field and click on Search to find the details of the MC and Approval Status.

If MC is not found, Could not find the details for application Id is displayed.

In the Medical Center Details Tab, Enter Medical Center Name, Registration No., Medical Center Address, Email, Fax No..

Select Working Hours on Weekdays and Weekends for the MC.

Click on Browse to navigate to the MC Logo image and select it.

Click ( ) Upload Logo to upload the image.

Click on Save to Save the MC details and navigate to the Next Tab.

In the Contact Person Details Tab, Enter the details of Owners and Representatives.

In the Doctor Details Tab, Enter the Doctor Details.

To upload Doctor's Signature, Click on Browse.

Click ( ) Upload to upload the image.

)save to save the doctor details.

Click on Add More Doctors to enter Another Doctor's Details.

When done entering doctor's details, Click on Save to Save the Doctor Details and navigate to the next tab.

In the Attach Documents Tab, Select Document dropdown has the list of all requisite documents to be uploaded for MC Registration.

Select the document description you want to upload, Click Browse to navigate the the file you want to upload and Click Upload Document.

You can save the Medical Center Details by clicking on Save.

To Submit you must check the checkbox at the bottom of the screen.

When you check, the Submit button appears. Click on Submit to Complete the Medical Center Registration.
